Victorian Service Coordination
Service Co-ordination aims to place consumers at the centre of service delivery – ensuring that they have access to the services they need, opportunities for early intervention and health promotion and improved health outcomes. The practice of service coordination particularly supports more effective ways of working with people with complex and multiple needs. For example, it provides a good foundation for the practice of integrated chronic disease management.
Service Co-ordination is facilitated by Primary Care Partnerships where agencies come together to agree on how they will co-ordinate their services so that consumers experience a health system that works together.
