Regional Wounds Victoria (RWV) – Grampians Region

RWV is a collaborative of Wound Clinical Nurse Consultants who target the management and treatment of chronic wounds within the Victorian Community.  RWV works together with staff in the Commonwealth Home Support Programme (CHSP) and HACC Program for Younger People (HACC PYP) funded district/community nursing services and public sector residential aged care services (PSRACS – high level care residents).

RWV – Grampians has 1 Nurse Consultants whose support includes:

* Providing an onsite and remote consultative service for wound assessment and treatment planning in consultation with nursing staff;

* Facilitating education, training, support and resources regionally;

* Providing access to clinical expertise across the region

Referrals may be received from nurses working within CHSP/HACC PYP and Public Sector Residential Aged Care facilities.
