Horsham Partnering Project
Key Project Strategies:
- To build closer links between HACC Assessment Services at Wimmera Health Care Group and Horsham Rural City Council
- To establish joint Living at Home Assessment processes between the two organisations
- To identify opportunities where HACC assessment, using the Active Service Model, can build on Integrated Chronic Disease Management work.
- To expand in-service training to assessment officers by local allied health practitioners from partnering community health services or district nursing services. For example, training in continence management, dementia, nutrition, functional assessment and mobility, all of which are relevant to the ASM approach.
- To investigate processes to decrease duplication and improve communication.
