Home and Community Care Program for Younger People (HACC PYP)
The primary outcome of the initial project (2012) was to improve access and choice for Aboriginal people living in the Grampians Region by maintaining links between the three Aboriginal Controlled Community Organisations (ACCO’s) and the seventeen HACC Assessment Services (HAS) in the area.
This project plan was based on the understanding that Aboriginal people have the opportunity to receive HACC services from an ACCO or HAS provider and, where required and preferred, both services.
Four regional plans were developed within the region with key issues and strategies being very similar. The four regional plans are:
- Ballarat and District Aboriginal Cooperative and Central Highlands HAS
- Budja Budja and Grampians Pyrenees HAS
- Goolum Goolum and East Wimmera HAS
- Goolum Goolum and West Wimmera HAS
The four regional plans were complemented by a final project report developed for the HACC Program, Department of Health, Grampians Region by Roger Hastrich, Consultant.
