A Grampians Region Sector Support and Development Team Program:
Community Nursing – Continence Nurse Advisor – Continence Nurse Consultant action plan
In 2015 this program commenced engaging with Community Nurses to develop clear pathways for working with Continence Care in the Community. The Home and Community Care Program funded a number of Continence Nurse Advisors/Consultants within the region and linked them in to our working groups.
The program has utilised the skills of this group to develop an Assessment and Screening Checklist for Community Nursing and Assessment Services along with a Practical Guide to Continence Services in the Grampians Region which was finalised in June 2017. This document incorporates the following information:
- Agreed role of Community Nursing, Community Support Workers, Continence Nurse Advisors/Consultants and Assessment Services
- Agreed referral pathways between key players
- Agreed assessment and screening tools for Community Nurses and Assessment Officers
