Welcome to this Allied Health page.  This page is available for clinicians within the region to add to with reports on their new and interesting projects and programs. 

There are quite a lot of Allied Health clinicians in the Grampians region who are funded by either the:

  • Commonwealth Home Support Programme (CHSP) or the
  • Home and Community Care Program for Younger People (HACC PYP)

The original Home and Community Care Program (HACC pre 1 August 2016) was very enthusiastic to ensure that Allied Health clinicians were welcomed to the community program and encouraged to see themselves as community clinicians.

Every opportunity was taken to engaged HACC funded clinicians in the community and draw effective working links between Community Support Workers and clinicians.

A number of ‘Guiding Principles’ were developed (with assistance and proof reading from the clinician) to promote effective working relationships between personal care providers and referring parties.  You will note that the tools were all written prior to Transition to the CHSP – therefore the HACC references remain but the guiding principles are still appropriate to the clinicians no matter who funds them:

Grampians Region Allied Health Forum:
