Nutrition Screening Tool – Investigating Capacity to Access Nutrition (ICAN)
This tool was developed through the following project: Eating for Independence, Implementing a nutrition pathway for identifying & assisting older people living in the community (Hume Region Home and Community Care Program 2016)
The tool developers are happy for the tool to be used by other agencies – but – you are not to add your own logo agency, adapt the tool content in any way and you cannot remove the HACC logo.
