Information About Me
The Information About Me handbook has been developed as a resource that promotes dementia friendly and enabling practices. We encourage the use of the handbook for people with dementia and their carers that are supported by HACC social support and respite programs. The handbook provides people with a place to record information that will be helpful to staff and volunteers and will ensure support is attentive to preferences, interests and routines of the individual. The handbook can be filled in manually or electronically but please note that photographs cannot be uploaded in the document at this stage.

The word version of Information About Me does have the capacity to type into it and to insert pictures. Instructions for adding a picture:
- Click and drag to highlight the “INSERT PHOTO HERE’ text
- Go to the tool bar at the top and click on INSERT
- Select PICTURE
- Select FROM FILE – click on desired picture and click on OK
- To centre the picture click on the centre table as per usual formatting
- Picture can be made larger or smaller by dragging from the corners
GP Dementia Pathways Tool
The GP Dementia Pathway tool has been developed as a key strategy of the Regional dementia project to build workforce capacity to better understand and respond to the needs of people with dementia. The pathway will benefit General Practitioners and Practice nurses by:
- providing clear consultation processes with decision points that will assist with assessment and management of dementia
- providing referral pathways to specialist dementia services such as the Cognitive Dementia and Memory Service and referral
requirements
While a valuable resource for primary health practitioners, the pathway will also benefit allied health professionals, assessment staff and community care staff. The pathway will build knowledge and understanding by:
- providing resources and information that raise the awareness and recognition of significant symptoms of dementia and emphasises the benefits of a timely diagnosis
- providing information on specialist and community services that can support the person with dementia and their care to live well in the community
The Grampians Region Community Aged Care Dementia Project is a Department of Health initiative that seeks to support the rationale outlined in the KPMG Dementia Services Pathways Guide 2011.
The Diversity framework recognises Dementia as one of the five key priority areas to be addressed in HACC Diversity planning. The project will assist organisations to develop strategies specific to dementia and ensure diversity planning for dementia is responsive and supports equitable, accessible and person-centred service systems.
Key Project Strategies
The project aims to inform and support service agencies, health professionals and general practitioners at local level by providing service pathways, support tools and recommendations that focus on best practice in care for the person living with dementia and their carers.
The HACC Scope and document current dementia services available across the Grampians Region
- Initiate, support and facilitate the implementation of a Regional Demential Advisory Group
- Develop a strategic regional plan to adapt the Dementia Services Pathways (KPMG Feb 2011)in Grampians Region
- Assist organisations to identify demential specific strategies to be included within HACC diversity plans
- Develop a community dementia resource and service provider regional toolkit
- Develop a web based dementia pathway for use by General Practitioners.
- Development will be in partnership with Grampians Region Medicare Local and CDAMS
- Seek opportunities to develop and enhance HACC Respite programs
