The Grampians Region Personal Care Protocol was first developed in 2009, reviewed in 2017, and again in 2019. It was first developed in response to requests from Grampians Region Home and Community Care (HACC) personal care providers for the provision of a document that clarified their role in the timely and effective delivery of personal care services.
This Protocol is not a Commonwealth Home Support Programme or Home and Community Care Program for Younger People policy document. It is an agreed protocol between funded service providers in the Grampians Region.
This protocol does not replace policies or procedures of funded organisations. Its content can be used to guide the development of organisational specific policies and procedures when providing assistance with medication.
This protocol does not determine the practice of registered nurses as their practice is determined by their individual registration as a nurse.
Aim of the protocol
The aim of the Grampians Region Personal Care Protocol is to support and guide the delivery of personal care services for both Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACC-PYP) providers.
